By signing this form, I grant Hayden Medical permission to use my testimonial, along with my name, title (if applicable), in various promotional materials. These materials may include, but are not limited to:

  • Website content
  • Social media posts
  • Brochures and flyers
  • Presentations
  • Advertisements

I confirm that the testimonial reflects my honest opinion and experiences with Hayden Medical.

I waive any right to inspect or approve the finished product or any material in which my testimonial appears. I also acknowledge that I am not entitled to any compensation for the use of my testimonial.
